If convenient, you may email or fax your completed Registration Form, and Nutrition History Form & Food Journal to the office two days before your scheduled appointment. - Nutrition History Form & Food Journal
- - Most recent lab work. Your physician may fax those results to me at 540-373-7008.
- A one hour initial consultation fee of $100.
24 Hour Cancellation Policy
If you are unable to keep your appointment, kindly give 24 hour notice or you may incur a $25 fee.
